Service Plan for Day One Integrative Services
The goal of the SUD Program is to teach clients skills to maintain sobriety. To accomplish this task, the services address major lifestyle, attitudinal and behavioral issues, which can undermine the goals of treatment or inhibit the individual’s ability to cope with major life tasks without the use of psychoactive substances. Individual and group therapy are used to support the clients in addressing substance use and mental health issues. The program offers evidenced based curriculum. Provided Services
Provided Services
Medical Services:
Medical services are those activities performed by a physician, PA, CNP, registered nurse or licensed practical nurse to address the physical needs of clients. Medical services may include health care examinations, health assessments, taking vital signs and reviewing laboratory findings. All medical services will be delivered by staff who are credentialed by the Ohio Board of Nursing or by the Ohio State Medical Board. Providers of medical services will be supervised by a registered nurse who is registered with the Ohio Nursing Board or by a Certified Nurse Practitioner who is licensed by the Ohio State Medical Board.
Urinalysis:
A urinalysis will be performed upon admission and randomly throughout the program to test an individual’s urine to detect the presence of alcohol and other drugs. Testing may include laboratory testing and/or urine dip screen. Each specimen will be collected in a manner to minimize falsification. The collection containers will be labeled to reflect the identification of the person from whom the specimen was obtained and the date that the specimen was obtained. Day One Integrative Services will provide urine dip screens and the results of the UDS will be reviewed by the program staff and a copy of the results placed in the client’s file. Positive results will be shared with the client and other members of their treatment team.
Program Services:
Day One Integrative Services will provide treatment services in the form of individual and group settings, based on the needs of the clients. This program is designed for individuals who may struggle with substance use disorders, criminal activity, mental disorders, impaired functioning and difficulty adapting to societal norms. Outpatient Services:
Day One Integrative Services will provide level one outpatient
services to clients who are in early stages of change, as a “step down” from more
intensive services; or for those who are stable and for whom ongoing monitoring or
disease management is appropriate. The services provided may include individual and
group counseling, motivational enhancement, family therapy, educational groups,
psychotherapy, or other skilled treatment services. Services will be provided at a
maximum of 9 hours per week by certified staff including CMS, QMHS, CDCA, LCDC II,
LCDC III, LICDC, LICDC-CS, LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, PRS, PRS-S.
Intensive Outpatient Services:
Day One Integrative Services will provide 2.1 level of care services that include individual and group counseling, educational groups, recreational therapy, psychotherapy, motivational interviewing, motivational enhancement and engagement strategies, and other skilled treatment services. Services will be provided at a minimum of 9 hours per week by certified staff including CMS, QMHS, CDCA, LCDC II, LCDC III, LICDC, LICDC-CS, LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, PRS, PRS-S
Partial Hospitalization Services:
The 2.5 level of care is intended to assist clients who are struggling with instability with medical and/or mental health conditions. In addition to the services offered in Intensive Outpatient Services, at this level of care Day One Integrative Services will also provide medical, psychological, laboratory, toxicology and emergency services. Supportive housing is also available. The services will be provided at a minimum of 20 hours per week by certified staff including CMS, QMHS, CDCA. LCDC II, LCDC III, LICDC, LICDC-CS, LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, PRS, PRS-S.
Individual Counseling:
Individual counseling will involve a face-to-face encounter or in some cases via telehealth video or phone call between a client or client and family member and a counselor. Special skills to assist an individual will be utilized to assist in achieving treatment objectives through the exploration of alcohol and other drug use and/or addiction and their ramifications. An examination of attitudes and feelings will be explored along with consideration given to alternative solutions and decision making and/or discussing didactic materials with regard to alcohol and other drug related problems. Individual counseling services will be provided at a program site certified by the Ohio Mental Health and Addiction Services.
Group Counseling:
Group counseling will involve the utilization of special skills to assist two or more individuals in achieving treatment objectives. Groups will explore alcohol and other drug problems and/or addiction and their ramifications, including examination of attitudes and feelings, alcohol and other drug related problems. Groups will focus on helping individuals increase awareness and knowledge, extent, and harm of their alcohol and drug addiction. Such groups will consist of lecture, viewing a video or a structured discussion session, and shall be documented. Groups will not consist of greater than one counselor to 12 clients. Group counseling will be documented. Group counseling services will be provided at a program site certified by the Ohio Department of Mental Health and Addiction Services.
Family Counseling:
At Day One Integrative Services, family counseling will utilize special skills in sessions with individuals and their family members and/or significant others under the guidance of a counselor. Family and relationship issues will be addressed related to alcohol and other drug abuse and/or dependence for the purposes of promoting recovery from addiction. Family counseling services will be provided at a program site certified by the Ohio Department of Mental Health and Addiction Services.
Crisis Intervention:
Crisis intervention will be conducted as face-to-face interaction with a client in response to a crisis or emergency situation experienced by themselves, a family member and/or significant other. An evaluation of what happened during the crisis will be completed along with the individual’s response or responses to it, including emotional reactions, mental reactions, physical reactions, and behavioral reactions. Information about the individual’s strengths, coping skills, and social support networks will also be obtained.
Crisis intervention services will be provided at a program site certified by the Ohio Department of Mental Health and Addiction Services. If an individual is determined to have unstable medical problems, he/she will be referred to a medical facility. If the individual is determined to have unstable psychiatric problems, he/she will be referred to a psychiatric facility. If an individual is experiencing withdrawal symptoms from use of alcohol and/or other drugs he/she will be referred to a person and/or entity that can provide the appropriate level of detoxification services.
Crisis intervention services provided by Day One Integrative Services will be provided by individual service providers who have current training and/or certification, which will be documented in their personnel files. Documentation will include CPR, First aid, and De-escalation techniques.
SUD Case Management:
SUD Case management services will be provided to assist and support individuals in gaining access to needed medical, social, educational and other services essential to meeting basic human needs, including interactions with family members, other individuals or entities. At a minimum, case management provided will include an assessment, development of a case management plan of care, referral, monitoring and follow-up.
Case management services provided by Day One Integrative Services will include coordinating client assessments, treatments, planning and crisis intervention services; providing training and facilitating linkages for the use of community resources; monitoring service delivery; obtaining or assisting individuals in obtaining necessary services, i.e., financial assistance, housing assistance, food, clothing, medical services, educational services, vocational services, recreational services; assisting individuals in becoming involved with self-help support groups; assisting individuals in increasing social support networks with family members, friends, and/or organizations; assisting individuals in performing daily living activities; and coordinating criminal justice services.
Case management will be provided at a program site certified by the Ohio Department of Mental Health and Addiction Services, in the community, or by telephone.
Peer Support Services:
Peer recovery services are community-based services for individuals with a mental illness and/or substance use disorder and consist of activities that promote recovery, self-determination, self-advocacy, well-being, and independence. Peer recovery services are individualized, recovery-focused, and based on a relationship that supports a person’s ability to promote their own recovery.
Peer recovery services may include, but are not limited to: Ongoing exploration of recovery needs; supporting individuals in achieving personal independence as identified by the individual; encouraging hope; supporting the development of life skills such as budgeting and connecting to community resources; developing and working toward achievement of personal recovery goals; modeling personal responsibility for recovery; teaching skills to effectively navigate to the health care delivery system to effectively and efficiently utilize services.
Peer recovery services will be provided at a program site certified by the Ohio Department of Mental Health and Addiction Services, in the community, or by telephone.
Population Served
The characteristics of the Day One Integrative Services population include: Those 18 years of age or older requiring services for addictive, substance related and co- occurring conditions that meet the ASAM LOC criteria for services offered.
Region Served
Day One Integrative Services primarily serves clients from the following counties: Clinton, Crawford, Hardin, Knox, Licking, Logan, Marion, Morrow, Perry, Richland, Union, and Wyandot.
Service Delivery
Services are provided in person and via telehealth. Day One Integrative Services utilizes HIPAA compliant software to ensure that client privacy and security are protected. Day One uses Doxy.me and Microsoft Teams software programs when providing telehealth services.
Frequency of Services
The frequency of service delivery is generally determined by the presented needs and the client’s individual program of recovery as depicted in the treatment plan. Treatment is offered based on their individual presenting needs at time of the assessment.
Referral Sources
Referral is through self-referral, Criminal Justice System, and other community agencies.
Admission Criteria Including Inclusionary and Exclusionary Criteria
Inclusionary Criteria:
Clients shall be admitted when:
- Physically stable.
- Determined to be at no or low risk of harm to self or others.
- Willing to participate in treatment.
- Severity of illness is low to moderate.
- Not active with another mental health or substance abuse agency.
- Client will be admitted based on the specific admission criteria
- Client is court ordered to outpatient treatment.
Exclusionary Criteria:
Clients shall not be denied admission or services based on age, race, creed, ethnicity, sex, color, national origin, marital status, disability, or the inability to pay. When an individual is found to be inappropriate or ineligible for admission to outpatient services, all efforts will be made to link them to the appropriate level of care (e.g., MH/SUD residential, other appropriate treatment provider, inpatient hospitalization, etc.) or to alternate, more appropriate community services. The individual will be provided with phone numbers, locations, directions and contact person if available. Written documentation on client’s inappropriateness or ineligibility will be maintained in the electronic file record.
Access Issues/Wait List Management Process
Access:
Day One Integrative Services will provide client access to care based on the availability of openings within 48 hours. Clients seeking immediate care will be offered community resources.
Wait List:
Day One Integrative Services may maintain a wait list on occasion. Date, Client name, phone number and address are taken. Clients will be called based on the date they were placed on the list. In the case of cancelations, clients on wait list will be given first option of appointment.
Referral Process to Other Services After Admission (Internal and External)
When the clinician and/or client identify that additional or alternative services (e.g., individual/group counseling) are necessary, the following occurs to ensure a barrier free transition for the client.
Internal Referrals:
- The clinician and client will discuss the service desired by the client or assessed by the clinician to be of benefit to the client.
- The Treatment Plan will be updated with the client’s signature to reflect the additional service.
- The primary clinician will make a referral to the supervisor of the program for the desired services and include a brief summary of the needs of the client.
- The supervisor will assign the appropriate counselor who will provide both the clinician and/or case manager along with the program manager/director with an initial appointment time.
Referrals to Medical Services:
Referrals to the agency CNP are made on behalf of the clients when it is determined through clinical assessment that the need exist, and the client agrees to the service. Priority medical appointments are available to those clients who need urgent care. Same day medical appointments are made available to evaluate clients at high risk for self/other harm.
External Referrals:
Defined as linking a client to additional services outside of Day One Integrative Services when it is determined that the client is unable to have all of his/her needs addressed at Day One Integrative Services and may need unduplicated adjunct services while receiving services at Day One Integrative Services. With external referrals the case remains open while the client receives adjunct services. When staff (clinician, case manager, peer supporter) refer clients to services outside the agency, the following procedural steps will be followed to ensure a barrier free transition for the client:
- The staff member and client will discuss the service need to determine the appropriateness of the external referral.
- The staff member will discuss the appropriateness of the external referral as well as the intervention/outcome with their supervisor. If the staff member’s supervisor
- agrees with the intervention/outcome the supervisor will coordinate with the appropriate department to complete the required steps for making the external transfer as follows:
- Progress note to reflect discussion with client.
- Collaboration with external providers should be conducted (and documented) to exchange information as indicated or allowed by consent.
- Release of information, as applicable
Referrals to other agencies if client requests a second opinion, request to seek treatment elsewhere, or when the appropriate level of care is not provide by the agency:
- The clinician shall give the client a list of area service providers, or will aid the client in locating a certified program near his/her home. It will be the client’s responsibility to contact the other program and notify Day One Integrative Services when he/she has an appointment with the other program.
- The referring clinician at Day One Integrative Services will follow the policy for Release of Client Information necessary to make the referral.
- When applicable, the clinician, case manager or peer support provider will assist the client to link with local consumer advocacy groups, consumer/survivor/ex-patient groups, self-help groups, other alternative treatment providers and other avenues of support needed by the client.
- If a level of care indicates detox or residential services, the primary therapist completes the assessment update, collects release of information, and coordinates the transfer. The client continues with services until their acceptance date has arrived for the program being referred to. All information regarding to transfer is discussed with the client prior to the referral and following referral. The process for returning to the program is also discussed with the client and the agency the client is being referred to.
Continued Stay, Discharge, and Other Termination Criteria
Continued stay criteria process is in accordance with the client treatment plan, and goal statement in transition plan.
Discharge criteria are in accordance with the client treatment plan and medical necessity.
Transfer of a client:
When a client is transferred from one level of care to another within the agency, an assessment update is completed discussing client progress and reason for change in level of care. The treatment plan is also reviewed at that time and any changes to the treatment plan are completed with input from the client. A transfer summary is completed with the client to identify progress and continued programming needs.
Discharge from services:
Discharges will be by mutual consent of staff and client, whenever possible. A discharge interview will be held with each client whenever possible. Discharge decisions shall be made collaboratively will all service providers involved, then will be reviewed with the Clinical Supervisor. The clinician shall notify all staff involved with services to the client. Clients can be voluntarily or involuntarily discharged when:
Involuntarily:
- No progress in service/treatment is attained due to client’s lack of adherence
- Client’s level of care requires a referral to services outside the agency and client is/is not in agreement with the referral
- Client demonstrates consistent failure to comply with all appropriate treatment service recommendations.
- Client has had no services or contact for 3 consecutive visits or 30 days without a valid absence
- Client is being served at another treatment facility for the same service
- Client is incarcerated for a period of 90 days or longer.
- Consent for treatment is withdrawn by the client.
- Client did not comply with and is in violation of program rules
- Client behaviors are a result of:
- Possession or use of alcohol and/or drugs on premises
- Distribution, Dispensing, Possession, Use of Controlled Substances, or Unsecured medication on Premises
- Weapons, fighting, threats or abusive behavior on premises
- Smell of alcohol on breath or signs of intoxication on premises
- Sexual harassment or acting out on premises
Voluntary terminations:
- Client has achieved all identified goals and objectives
- Client prefers to discontinue services
- Client feels they can attain more appropriate service/treatment elsewhere
- When client request services that are no longer/not offered by the agency
- Client has 3 consecutive missed appointments without notice
- Death
Discharge procedures (planned and unplanned) shall include the following:
- A documented conference via progress notes between therapist and client if available to discuss discharge, post-discharge, transition and follow-up plan as applicable.
- All documentation concerning discharge shall be completed within 10 calendar days from last date of contact with the client.
- Copy of discharge plan is given to the client or if not present, mailed to client.
- Discharge plan contains any referral information with phone number and address of the referral contact.
- Client is also given or mailed a copy of medication list.
